2 Investor Account Opening Form Request to open a Trading Account Number with a broker licensed on Bahrain Bourse. Investor should have "Investor Number - Form 1" before obtaining Account Number with a broker  
3 Securities Deposit Form Request to Deposit physical shares certificate(s) of an investor into his/her/it account with the CSD system  
4 Securities Statement Request Form Request to get a printed Statement of Investor Accounts  
5 Securities Transfer Form Request to transfer Securities between an Investor's Account
